This year, as part of our School Improvement Plan, we are participating in the Catholic Dialogue Schools Project, a major research project facilitated by the Catholic University of Leuven in Belgium. This involves the collection of authentic data from our staff, Year 5 and 6 students and school community and will ultimately help us identify and strengthen our identity as a contemporary Catholic School. Your participation in this process is critical to providing us with insight into our St Matthew’s School community. Your religious background or knowledge of religion does not matter, and your answers are completely anonymous.
The survey contains four sections and will take approximately 45 mins to complete. In our busy lives, we acknowledge that this is a big ask! As an incentive, we are providing a movie ticket lucky draw each week with the grand prize of a car park space for your use for the remainder of the year drawn just prior to Easter. To be included in this draw, email a screen shot of the completion certificate to email@example.com, please include your name and year level of your eldest child in the subject line. Alternatively, send in the printed certificate indicating successful completion along with your details to the office. The survey closes on 29 March and is open to all parents/carers in our community.
OUR TIP: Some questions are deliberately written in a way that may challenge your thinking or you may find yourself rereading them to gain clarity. We would encourage you to answer with your first instinctual response, as this often indicates your most honest answer. Some questions appear to be repeated. However, you are asked to identify what you currently see, and what you would like to see in your ideal school.

Enter the website access password Ghj654
On the main portal, click ‘to the surveys’
Under ‘User Access’ click on ‘New User’
Enter the Entry Code X26y7k in the field labelled ‘Group entry code’
You will be given an individual user code and password. Please take note of both of these codes and keep them for future reference. These can be printed by clicking on the relevant button.
You will now be presented with the preliminary Identification Diagram and Profile Questionnaire. Once you complete these you will arrive on the main questionnaire page, where you must click on each questionnaire and answer the questions to advance to the next questionnaire.
Request a ‘certificate of successful participation’. Email this to firstname.lastname@example.org or send it to the office for inclusion in the weekly prize draw. Please include your name and the year level of your eldest child.
